Established in 1981 through a contract with DPH&SS,
Division of Senior Citizen (Title III-B) of the Older Americans Act of 1965
as amended in year 2000 to provide services to seniors 60 years and older.

CMS receives referrals from the general public; determines
eligibility and provides a comprehensive assessment of the seniors’ needs and
arrange an optimum package of services to fulfill those needs.
To be eligible for the program applicants must be 60 years or older; are vulnerable or “at risk” of institutionalization; or have difficulties with activities of daily living; or are in need of multiple, on-going and long term services; or have little or no support of close family and friends. (Note: For home based services, clients must have
two Activities of Daily Living Skills Impairments.)
Provides information and assistance to the frail homebound seniors
and their families to access the available services in the community such as:
Congregate Service: Senior Center
Operation Transportation Congregate meal Home based
Services: Homebound meal
delivery Adult Day Care
Service In-Home Service Assistive Transportation
Caseworker is assisting client with
her Social Worker Visits His Client. Shopping and managing her money.
