HISTORY

 

CHP was established in 1996 under a Memorandum of Agreement with DISID and Catholic Social Service to serve 15 significantly disabled persons in improving their Activities of Living Skills functional abilities.

 

STATISTICS

 

Fifteen is the contracted number of consumers at 100 hours a month or a total of 1500 aggregate hours monthly.  CHP serves 14 full-time consumers and 4 part-time consumers for a total of 18 served.

 

HOURS AND LOCATION

 

CHP hours are from 8:30 am to 1:30 pm

The center is located at 235 Westbrook St. in Barrigada.

 

 

 

 

The purpose of the program is to provide independent living services training, socialization and integration to the significantly disabled adults who are not eligible for services who fall between the cracks in the community.

 

Each consumer has written goals that are worked on daily that include activities of daily living skills (ADL) training to become independent.  Basic skills are taught that include learning to recognize the letters of their names/write their names, count, identify colors and shapes.  They are also taught to recognize public figures in the community to include police, nurses as well as their family members, identify recreational areas as well as business establishments.

 

 

 

Consumer Does Gardening

 

Additionally, they are taught pre-vocational/domestic skills and include cooking/preparing simple meals and sandwiches, sweeping, mopping, cleaning bathroom, washing clothes, washing and drying dishes, and wiping tables.  They are engaged in gardening and yard cleaning. Consumers working on their routine activities or schedules to retain their learning skills, academic skills such as reading and writing, and budgeting/budget management training.

 

They participate in recreational and leisure activities that include weekly outings in the community as well as celebrating their birthdays.
