HISTORY On October 2, 1995, Catholic Social Service opened its doors to provide adult day care services through funding by the Dept. of Public Health and Social Services (DPH&SS), Division of Senior Citizens (DSC). STATISTICS
Total Capacity is thirty-five (35) elderly clients. “God
made us in his image . . . Catholic
Social Service is guided by this teaching as we serve those in need with
dignity and respect.”
Participants
work on several center activities.
They are assisted by caregivers. Adult Day Care I serves the
northern area’s elderly who are sixty (60) years and above who suffer health
impairments. Impairments such as Parkinson’s, wheelchair user, amputation,
sensory loss, blindness, incontinence, diabetes, etc. The Adult Day Care Service are staffed
by professional and technical assistants. The programs provide a safe
environment, proper nutrition, adequate personal hygiene, adequate exercises
and activities.
Referrals are received through Case Management. Assessment
are forwarded to Adult Day Care. The
Adult Day Care Services are opened from 7:00am to6:00pm Monday thru Friday and
offers free quality services up to ten (10) hours daily. Activities such as
educational, social, recreational, spiritual and therapeutic are provided to
meet the needs of each individual client. A
well-balanced meal that meets 1/3 or RDA requirements for adults provided
daily at no cost. However, a $3.00 charge per day is assessed to each elderly
who choose to partake with morning and mid-afternoon snacks, and/or bring
snacks from home. The program provides daily activities such as morning rosaries, 3 pm Divine Mercy, morning and afternoon exercises, and arts & crafts. Local
and world news is read daily from PDN.
They conduct trivia questions and answers, monthly field trips, center
backyard BBQ, socialization, card games, bingo and we celebrate Christmas
with a big party. The elderly participates on arts and crafts and other
activities. Field trips are scheduled to have the elderly enjoy recreational
facilities and an occasional opportunity to enjoy buffet luncheon at local
hotel restaurants. An elderly client is shown bowling which is one of the
once a month activities in the community. WHAT SERVICES
DOES ADULT DAY CARE PROVIDE? Catholic Social Service Adult Day Care Program is an out-of-home respite
type program for adults who are unable to function at home without supportive
services and who do not need 24-hour care.
These are persons who would normally be homebound and have no one at
home during the day to look after them. The Adult Day Care Program is
available for adults, age sixty (60) years and older, who suffer health
impairments such as sensory loss, loss of mobility (caused by strokes,
amputation, diseases such as arthritis, Parkinson’s, poor circulation,
injuries and blindness) incontinence, obesity and diabetes. |
ADMISSION
POLICY Case
Management Referral and Assessment.
An Intake, Profile and Referral form and assessment must be
initiated by the Case Management Program to determine eligibility prior to an
individual being admitted into the program.
TRANSPORTATION SERVICES For those individuals requiring transportation to and from the LUNCH/SNACKS A well balanced lunch that meets 1/3 of the Recommended Daily
Allowance (RDA) for adults are provided at the center at no cost to the
family. Mid-morning and afternoon snacks are also provided for a small fee
and/or clients can bring snacks from home. The Adult Day Care Program offers free quality care, for up to ten
(10) hours daily, with activities that are planned to fit individual
capabilities. These activities take into consideration the overall needs of
the clients.
RECREATIONAL THERAPEUTIC ACTIVITIES
PROVIDED AT THE The Adult Day
Care Program provides social, educational, recreational, therapeutic, and other
activities on a daily basis. The activities
provided at the center are determined by the participants needs abilities and
interests and are performed in a professional, courteous, safe and caring
manners. The Adult Day

Snack Late Afternoon Activity Session Center participants are assisted
through activities at his or her own pace. An activity will cease when the
